Director of Prevention- Responsible for the supervision and training of all personnel and programs. Oversees the quality and delivery of client care for EHEAL’s clients. Ensures quality client care remains compliant with regulatory requirements, professional standards, and the facility’s policies and procedures. Takes responsibility for tracking and documenting services, ensuring reports and spreadsheets are completed and returned in a timely manner. Works closely with community agencies. Generates creative ideas and solutions to problems; able to think outside the box. Analyzes problems systematically and logically; and is resourceful when developing and implementing solutions. Participates in trainings/meetings as required by contracts and the Executive Director. Provides pertinent program feedback to Director. Is responsible for planning team meetings, schedules, and program quality activities for prevention/intervention programs.

Navigator- The position is responsible for the planning and implementation of all prevention and treatment services related to ending the HIV epidemic including, but not limited to education assessment, evaluation, enrollment, data collection, financial assistance, and follow-up. Under the supervision of the Prevention Director, the Navigator will be responsible for helping clients navigate and access HIV testing, PrEP treatment and care, and other community resources..

Peer Support Specialist- 

The peer navigator will serve as an empathic, trustworthy, knowledgeable, and reliable resource for the client's HIV care and help with support services while maintaining confidentiality at all levels.

RESPONSIBILITIES

The responsibilities of the Peer Navigator include, but are not limited to:

Accompany clients to HIV medical care visits and to other non-medical care visits (if requested).

Facilitate the clients' linkage to and retention in HIV care (including reengagement of clients previously lost to follow up).

Serve as a liaison between the client and the clients' healthcare providers and housing provider.

Serve as a member of the treatment team, coordinating efforts to support the client with case managers.

Build effective working relationships with clients, case managers, and providers to improve coordination of HIV care and supportive services.

Record and track client information securely and confidentially with HMIS.
